I don't think there's any prohibition on a doll with a hijab - the prohibition is on images representing Mohammed, AFAIK.

MMM Probably because figures can be used in forms of black magic too.
The idea is that 'we're' making copies of God's great handiwork or summat or other ?!?!?
As if Mohd was anything special anyway. Muslim seem to think he was some kind of, almost, demigod, by the way he's treated in Islam.

I'm not certain that this is necessarily the case. I think that is more likely that some traditions in Islam forbid pictorial depictions of the prophet and some do not - rather like the traditions of imagery in RC and Orthodox church buildings compared with those of protestant sects.
If you Google "images of mohammed" you will get a very rich collection of images.

My first thought on looking at the picture was The Mummy (https://pmcdeadline2.files.wordpress.com/2017/06/the-mummy.jpg?w=446&h=299&crop=1).

I wonder it is would be anti islamic to put underwear on the doll? Would be anti islamic not to give her underwear?

She might come already fitted with underwear but will need a change so, yes, I think underwear will be put on her. Muslim women wear underwear you know :).
