Hi all

This programme started on BBC2 last night and gives an insight into the workings of a large NHS Hospital Trust in Nottingham. And yes this happens to be the Trust I work for, so lots of my colleagues appear on this programme, a few of whom I know very well.

Interesting and worrying viewing imo.
Before we work on Artificial Intelligence shouldn't we address the problem of natural stupidity.

Depressing. :o May one enquire what is your role within the NHS?

What needs to change, Trent? My impression is that a lot of the problems are down to a lack of social care for people when they are in a position to leave, but there must be other things need fixing. How much of a problem are the private finance initiatives so beloved of Blair/Brown?

I don't think that QMC is a PFI hospital. It was built - and was the largest purpose-built teaching hospital in Europe - long before PFI was invented.

I think the real problem lies in the statement by one participant in the programme to the effect that the hospital treats people not targets. The micro-management of the NHS by central government is efficiency driven not effectiveness driven.

As HH says very little impact of PFI on QMC, and what there is was relatively well planned for in economic terms. Under capacity is a huge problem or rather using all our capacity all of the time. We've been running at over 95% bed occupancy for months now. Most modern healthcare system aim at between 75 - 80% as the optimum occupancy.

To alleviate that, social care has to be addressed. It is crazy to keep "well" patients in bed that costs £380 per night when the cost in a care home is about a third of that price. However, without a properly funded care home system that isn't only being squeezed at the margins, but at its very core by local govt; who in turn are being squeezed by central govt; there seems no sensible way forward on this under current policy.

Long term we need the population as a whole to take much more responsibility for their own health. Not easy when big business in this country is determined, for example, to make us consume more and more useless calories in their pursuit of profit.

My oft repeated mantra about this government holds spectacularly true in this case; they know the cost of everything and the value of nothing.

I have just watched episode 2 of this series.

Interesting situation:

David Grant, an overworked head-and-neck cancer surgeon is leaving Nottingham to join his wife in the USA. His departure will potentially leave an understaffed department in an untenable condition.  His wife (a plastic surgeon, apparently) would earn £77,000 in the NHS - in America she can earn $500,000.

In England he is treating all-comers on the basis of need. In the USA she will be treating only people who are adequately insured or people who can afford to pay.
