How do the various roles in a US 'Cabinet' get to be appointed as opposed to nominated?
Are your, or your friends'/relatives', garages, lofts or sheds full of unused DIY gear, sewing/knitting machines or fabric and haberdashery stuff?

But the article refers to X being appointed and Y being nominated by Trump.  What is the way that decides which system is used?

The appointees are White House staff, not part of the Cabinet.
